After a week of non-use the StarTeam server wrote this log when trying to log in from clients:

23 2000-04-28 16:59:37 E Exception caught in :
24 2000-04-28 16:59:37 E EXCEPTION during Server command
Exception class: CDBException
Exception code: -1 (FFFFFFFF)
Exception message:
Command ID: 2 (00000002)

Repeated for each failed login attempt. After restarting the NT Service all was well. We have seen similar messages when our DB server went down so is this also related to the DB (this time the DB server was up all the time).
